Steps to Register for PML Trip
Step 1 – Complete registration form for each traveler. (Available in OLLI Office or can be
printed from website by clicking red button below).
Step 2 – If paying with Credit Card, complete the “Credit Card Authorization Form”
completely. Include the following information:

Step 3 – If paying with check, fill in all necessary information with check made out to
“PML Travel and Tours”. Include the following on your check in the memo line: Contr. 24.213
Step 4 – If purchasing travel protection, which should be purchased at the time of
deposit, the cost is $92.00.
a. If payment with Credit Card, this can be added to the cost on the credit
card authorization form.
b. If paying by check, make out a second check to PML for the trip
insurance.
Step 5 – If not purchasing insurance, must complete the Travel Protection Plan Waiver.
